We are so glad you're interested in enrolling at CCPA! You'll quickly find our learning environment will empower your student to be his or her best.

The steps to enroll at CCPA include:

Complete an application. Click to access the form.
Provide the required documentation.
CCPA will use a lottery process to fill any available space in the desired grade level.
If you are chosen in the lottery, the parent/guardian will be contacted to finalize registration.

Please complete this form with your contact information, and a staff member will be in touch with you. We can set a time for you to complete an application in person, or an application can be mailed to you.

Additional Ways to Complete an Application

A paper application may be completed in person at 1425 Linn Street, during business hours.

Required Documentation

Along with your application, we will need the following documentation:

Proof of address (i.e. current utility bill, current lease, bank statement, etc.)
Social security card and birth certificate of each student enrolling
Most recent report card or transcript (for those previously enrolled somewhere else)
Current vaccination record
State-issued ID of parent or guardian
Proof of guardianship, if applicable

Having these items available upon the return of your application will streamline the enrollment process.
